Job Summary

Our client is seeking a dedicated Hospice Triage Nurse to efficiently manage daily tasks including reviewing schedules, monitoring inbox and voicemail, and triaging patient messages. This role requires effective communication with patients and infusion nurses, handling testing results, and addressing urgent patient concerns in a timely manner.

Responsibilities / Job Duties

• Review daily list for scheduling conflicts and unsigned or missing orders.
• Route and manage pending orders.
• Monitor inbox to review lab imaging results and inform patients of testing results.
• Triage and respond to patient messages.
• Respond to urgent calls and appropriately triage calls to PSR.
• Conduct rounds on patients receiving routine scheduled treatments in the infusion center.
• Make phone calls to new treatment patients from the previous day.
• Communicate with infusion nurses regarding acute issues.
• Perform end-of-day rounds to address any issues and prepare for the next day's activities.
