Position: Licensed Master Social Worker (LMSW) with Dialysis Experience
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) with dialysis experience to join our team. While candidates with dialysis experience are preferred, we will consider new graduates who have completed their Master's degree.
Shift and Status:
Full-Time position
Competitive hourly rate: $30 - $40 per hour
No weekends, no holidays
Position Summary:
The LMSW will provide social services to patients undergoing dialysis treatment. The role includes offering emotional support, counseling, and advocacy services to patients and their families. The LMSW will also assist with coordinating resources and services, ensuring patients’ needs are met during their dialysis treatments.
Duties and Responsibilities:
Provide counseling, crisis intervention, and emotional support to patients and their families.
Conduct psychosocial assessments to understand patient needs and develop appropriate care plans.
Advocate for patients' needs within the healthcare team.
Assist in resolving issues related to treatment, finances, and social concerns.
Develop and implement care plans for patients, focusing on improving their overall well-being.
Coordinate referrals to community resources and other supportive services.
Facilitate communication between patients, family members, and healthcare providers.
Educate patients and families about coping strategies and the emotional aspects of dialysis treatments.
Document all patient interactions and interventions in the medical record in compliance with organizational policies.
Requirements and Qualifications:
Master’s degree in Social Work (MSW) required.
Current and valid LMSW license.
Dialysis experience is preferred, but we will consider new graduates.
Excellent communication skills, both verbal and written.
Ability to work independently and as part of a team.
Strong organizational and time management skills.
Compassionate, patient-centered approach to care.
Familiarity with community resources and patient advocacy.
Physical Demands:
Position involves standing, sitting, and walking.
Occasional lifting of up to 10 pounds.
